Tamper-Evident Audit Log
Every PHI access is written to a SHA-256 hash-chained audit log. Modify any past entry and every subsequent hash becomes invalid, verifiable with one click. Satisfies ONC §170.315(d)(2).
Per-Tenant Data Isolation
Each facility lives at its own subdomain with strict tenant_id scoping enforced on every database query. No facility's data is ever co-mingled with another's.
